📌 Please verify your name, date of birth, and personal information.
📌 Please, enter your U.S. SSN (Social Security Number) or ITIN (Individual Taxpayer Identification Number). If you do not have SSN or ITIN, please select “No” and proceed to next questions.
📌 Fields “IIE identification number”, “FNR ID”, “Program Type” and “Campus” would be pre-populated and/or locked for editing by IIE.
📌 If the SEVIS ID field is blank, find the number at the top right of your DS-2019. This field is required only if you haven't entered a U.S. SSN or ITIN; otherwise, you can leave it blank.
📌 If you have a tax ID from your home country, enter it in the "What is your foreign (home country) tax identification number" box. If you do not have one or are unsure, leave it blank.
📌 If you are married, to a foreign or US citizen, please select “Yes” to question “Are you married?”.
📌 If you select you are married, please also make a selection whether your spouse is a US citizen or resident alien.
